Healthy Age Friendly Homes

Healthy Age Friendly Homes supports people to independently live longer and healthier lives in their homes. It does this by ensuring they are in an accessible, safe and warm home while feeling part of their community.

How does it work?

A Healthy Age Friendly Homes Coordinator will assess what supports are required to help you live independently in your home for longer. After this assessment, a person-centred support plan will be developed for you. 

Coordinators give information on adapting your existing home and any grants which may be available to you. 

Coordinators help you access housing, health and social care supports. These supports can help you decide on what you need to live the life you want in your community

If you feel your home is no longer suitable and wish to move, the Healthy Age Friendly Homes Coordinator can give you information on accommodation in your community and county.
